The coverage rules come from the law and the Bureau of Internal Revenue's Revenue Regulations 7-2010, which extend the medicine discount to prescribed vitamins and to supplies such as eyeglasses, hearing aids and wheelchairs. The FDA's Advisory 2024-0429 reminds drug outlets that over-the-counter medicines need no prescription for the discount and that all drug outlets and hospital pharmacies must grant it. The 5% grocery discount and its ₱125 weekly cap, and its extension to online purchases, are in the DTI, DA and DOE Joint Administrative Order 24-02 of March 2024, which also lets a representative buy with a signed authorisation valid for seven days.

How do pharmacies and hospitals compute it?

The discount is taken on the price net of VAT, because the sale to a senior or PWD is VAT-exempt. In RR 7-2010's own example, ten tablets at a VAT-exempt ₱5 each come to ₱50, less 20% is ₱40 payable. When the shelf price includes VAT: divide by 1.12, then multiply by 0.80. So ₱1,000 becomes ₱892.86, then ₱714.29; ₱350 becomes ₱312.50, then ₱250. On a hospital bill the order is fixed by PhilHealth's Advisory 2023-0036: VAT exemption first, then the 20% discount, then the PhilHealth case rate, and the PhilHealth benefit may not be reduced because of the discount; hospitals that fail to deduct the discounts before discharge face penalties up to suspension of accreditation (PhilHealth). With an HMO, the discount still applies to the bill and the HMO pays the reduced balance. The "no double discount" rule in the RA 9994 implementing rules means you cannot stack the 20% on a promotional price, and a senior who is also a PWD uses one ID or the other, not both.

Which ID do you need?

  • Senior citizens: the ID issued by the Office for Senior Citizens Affairs (OSCA) of your city or municipality, honoured nationwide, or any government ID showing you are a Filipino aged 60 or over (RA 9994, Section 4). The purchase booklet is no longer required for medicines and medical devices since DOH Administrative Order 2024-0017 (GMA News, December 2024); it is still used for the 5% grocery discount.
  • PWDs: the PWD ID issued by the city or municipal mayor or the barangay captain, or a passport (RA 10754). Apply through your city or municipal government, usually its Persons with Disability Affairs Office.
  • Buying for someone else: bring the senior's or PWD's ID and, for medicines, the prescription in their name; the discount is for their exclusive use.

What if you are refused?

Note the establishment, branch, date and the name of the staff member, keep the receipt, and complain. For seniors, the OSCA's legal duty under RA 9994 is to help you file the complaint with the Department of Justice or the prosecutor's office; for PWDs the route is the city's Persons with Disability Affairs Office; for drug outlets the FDA also acts, and for the 5% grocery discount JAO 24-02 sends complaints to the OSCA, the PDAO or the city social welfare office. The penalties in Section 10 of RA 9994 are two to six years' imprisonment and a fine of ₱50,000 to ₱100,000 for a first offence, ₱100,000 to ₱200,000 for a repeat, and revocation of the business permit. What else do seniors get?

Indigent senior citizens receive a ₱1,000 monthly social pension from the DSWD, the minimum set by Republic Act 11916 in 2022 (DSWD Social Pension); senior citizens are PhilHealth members, and every member registered with a YAKAP clinic gets free consultations, 13 laboratory tests and up to ₱20,000 a year of medicines at accredited pharmacies, on top of the 20% discount on anything they buy themselves. Remove the 12% VAT first, then take 20% off. Divide the VAT-inclusive price by 1.12, then multiply by 0.80. A medicine priced at ₱1,000 with VAT becomes ₱892.86 without VAT and ₱714.29 after the 20% discount, a saving of ₱285.71 or about 28.6%. Do PWDs get a discount at hospitals?
Yes. Republic Act 10754 gives persons with disability at least 20% discount and VAT exemption on medical and dental services, diagnostic and laboratory fees such as X-rays, CT scans and blood tests, and the professional fees of attending doctors, in all private hospitals and medical facilities, and the same in government facilities under DOH guidelines. Can a senior citizen get a discount on lab tests?
Yes. Diagnostic and laboratory tests requested by a physician, including X-rays, CT scans and blood tests, are subject to the 20% discount and VAT exemption in private hospitals, clinics and laboratories, and are free in government facilities under Section 4 of Republic Act 9994. The test must be ordered by a doctor as necessary for diagnosis or treatment.
Is the senior discount applied before or after PhilHealth?
Before. PhilHealth's Advisory 2023-0036 tells accredited hospitals to apply deductions in this order: the 12% VAT exemption, then the 20% senior citizen or PWD discount, then the PhilHealth benefit, and the PhilHealth benefit may not be reduced because of the discounts. Hospitals that fail to deduct the discounts before discharge face fines and suspension of accreditation.
Do I still need the purchase booklet for medicines?
Not for senior citizens. DOH Administrative Order 2024-0017, signed in December 2024, removed the requirement to present the purchase booklet when buying prescription medicines, over-the-counter medicines and medical devices; the OSCA-issued senior citizen ID, or another government ID showing age and citizenship, is enough. The booklet is still used for the separate 5% discount on groceries under the DTI rules.
